STEWART COUNTY SCHOOL DISTRICT SCHOOL-BASED HEALTH CENTER JUNE DENTAL SERVICES
Ms. Kayla, the School-Based Health Clinic Dental Hygienist, is taking Dental Services appointments for Wednesday, June 10th and Wednesday, June 17th at the School-Based Health Clinic on our school campus. These appointments are for all Stewart County School District students, summer school students, and summer break students.
Services provided at these visits:
Dental Examination/Cleaning: Evaluates overall oral health, typically involves a professional cleaning, an oral cancer screening, diagnostic X-rays, and an assessment of tooth decay and gum disease.
If your child already has a consent on file, please call Ms. Stephanie (temporary intake staff) at 229-545-7231, Ext. 2011 to schedule the appointment for your child.
If your child does not have a consent on file, please use the QR code below. Once you have completed the consent form and submitted it, Ms. Raquel or Ms. Stephanie will call you to schedule the appointment.
While you are completing the consent form, you can also give consent for other services that will be available at the School-Based Health Clinic in the coming weeks. Once the consent form is on file, you will only need to make an appointment for the other services.
